You must register to complete the enrollment questionnaire.

The enrollment questionnaire collects the information required by workforce development funders who pay for training. Upon completion of the questionnaire, Apprenticeship Institue reviews your information and performs the necessary criminal background check (Allied Health only). When we are satisfied your enrollment questionnaire has the necessary information, we forward your form to funders for review.
